About the role

  • Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions and determine physical, mental, and social needs
  • Take history, examine, determine diagnoses, and provide written documentation of patient visit per NCQA standards
  • Take patient vital signs as necessary and place case management referrals
  • Communicate with PCPs, patients, caregivers, agency nurses, other providers and vendors to assure proper diagnosis and care coordination
  • Perform all clinical duties while observing OSHA Universal Precautions and maintain patient confidentiality
  • Attend required meetings and in-services, participate in committees, and engage in professional development to maintain licenses and affiliations

Requirements

  • Active/unrestricted nurse practitioner license to practice in coverage states
  • Board certification in one of the following: ANCC, AANP or NCCPA
  • Active CPR Certification
  • Current enrollment in Medicare/Medicaid
  • Must maintain a valid driver’s license and good driving record
  • Outstanding EHR skills
  • Ability to conduct in-home visits and telehealth
  • Ability to take history, examine, determine diagnoses, and provide written documentation per NCQA standards
  • Ability to take patient vital signs as necessary
  • Ability to place case management referrals and communicate with PCPs
  • Ability to communicate with patients, caregivers, agency nurses, other providers and vendors
  • Perform clinical duties while observing OSHA Universal Precautions and maintain patient confidentiality
  • Ability to ride in automobile or van up to 150 miles daily in urban and/or rural settings
  • Regular lifting/carrying items weighing up to 50 pounds; full range of body motion required
  • Manual and finger dexterity, eye-hand coordination, normal visual acuity, normal hearing, standing, bending, walking and stair climbing
  • Geriatric training/experience (Preferred)
  • Skill in teamwork and maintaining effective working relationships with patients, medical staff, and the public (Preferred)
