Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions and determine physical, mental, and social needs
Take history, examine, determine diagnoses, and provide written documentation of patient visit per NCQA standards
Take patient vital signs as necessary and place case management referrals
Communicate with PCPs, patients, caregivers, agency nurses, other providers and vendors to assure proper diagnosis and care coordination
Perform all clinical duties while observing OSHA Universal Precautions and maintain patient confidentiality
Attend required meetings and in-services, participate in committees, and engage in professional development to maintain licenses and affiliations
Requirements
Active/unrestricted nurse practitioner license to practice in coverage states
Board certification in one of the following: ANCC, AANP or NCCPA
Active CPR Certification
Current enrollment in Medicare/Medicaid
Must maintain a valid driver’s license and good driving record
Outstanding EHR skills
