About the role

  • Provide 1:1 intensive care for patients in a calm home environment. Collaborate with healthcare professionals to ensure the best patient care and support.

Responsibilities

  • Individual work in a comfortable private household — not route-based care; you will care for a single patient
  • You provide 1:1 care in a calm, low-stress environment directly in the patient’s home
  • You can provide comprehensive, holistic care in coordination with your team, our nursing management (PDL), physicians and therapists, giving the patient your full attention
  • Communication and coordination with therapists
  • Implementation of rehabilitative measures and social support
  • You are the primary contact person for patients and their relatives
  • Joint activities and outings with our patients and, if applicable, their relatives
  • Maintaining nursing documentation

Requirements

  • You are a qualified geriatric nurse, registered nurse, licensed nurse, or pediatric nurse
  • Alternatively, you are already a specialist for out-of-hospital intensive care, a care expert for outpatient intensive and ventilator care, a specialist nurse in intensive care/anesthesia, or a respiratory therapist
  • You have an interest in providing outpatient care to patients requiring intensive care
  • You bring passion for 1:1 care
  • Your working style is conscientious, independent and responsible
  • You have empathy and provide attentive, compassionate care to your patient
