About the role
- Human Services Program Consultant conducting disease prevalence investigations and public health awareness. Performing interventions under the STD Program while collaborating with healthcare providers and community organizations.
Responsibilities
- Investigating the distribution and determinants of disease prevalence in populations
- Providing direct disease and public health awareness intervention activities under the auspices of the Sexually Transmitted Disease (STD) Program
- Conducting personal interviews with patients infected with a Sexually Transmitted Disease
- Educating patients regarding their infections and appropriate medical care
- Developing plans to locate and refer their sexual and needle-sharing partners to a medical facility
- Performing rapid field follow-ups to locate and refer infected persons
- Linking at-risk individuals to PreP services in the community
- Conducting educational presentations and in-service training as needed
Requirements
- Valid driver's license
- Access to private vehicle to perform work related duties
- Two years of experience working with a disease control program, social services program, or community-based organization
- Preferably bilingual in English/Spanish or English/Creole
