Case Manager II providing enhanced care management for high-need clients at Family Health Centers of San Diego. Focusing on comprehensive assessments and collaborative care plans to support the medically underserved.
Responsibilities
Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards.
Assists in preparing reports as required.
Conducts initial and ongoing assessment of client's health and/or support service needs.
Sets level of client need.
Creates formalized case plans and goals with clients; clients are high-need and require more intense services.
Develops a written care-plan with the client, identifying problems and needs.
Updates plan as client's needs change.
Provides therapeutic interventions to clients.
Provides intensive individual support to high need clients.
Requirements
3 years of work experience providing human services to high risk, medically underserved, disabled populations, and/or relevant community health populations required.
Bachelor's degree in Social Science field, Public Health, Healthcare Administration, or closely related field required.
Master's degree in relevant field preferred.
Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training required.
Must have a valid California driver’s license, an automobile, and proof of minimum levels of car insurance as required under California law, although limits of $100,000 are recommended.
Ability to work well in both a team-based environment and independently.
Basic computer literacy ability to comply with department needs and expectations.
Multi-lingual preferred, English, Spanish, Arabic, and Somali depending on current priorities and locations.
Demonstrated ability to be culturally sensitive and respect diversity.
