About the role

  • Care Manager coordinating healthcare services for members, collaborating with care teams and support systems. Focused on improving member quality of life and health outcomes through personalized care.

Responsibilities

  • Collaborates with members of an inter-disciplinary care team to improve quality and meet individual needs
  • Facilitates communication and coordinates care through assessments and individualized care planning
  • Engages members and natural support systems using strength-based assessments and trauma-informed care
  • Identifies and manages barriers to care plan goals
  • Educates members about treatment options and community resources

Requirements

  • Nursing degree from an accredited nursing program or Bachelor’s degree in a health care field or equivalent years of relevant work experience
  • Advanced degree associated with clinical licensure is preferred
  • A minimum of three (3) years of experience in nursing or social work or counseling or health care profession
  • Three (3) years Medicaid and/or Medicare managed care experience is preferred
  • Strong understanding of Quality, HEDIS, disease management, supportive medication reconciliation and adherence
  • Intermediate proficiency level with Microsoft Office, including Outlook, Word and Excel
