About the role
- Hospice Intake Manager coordinating initial client information and care continuity for JourneyCare Hospice. Ensuring proper documentation and collaboration with healthcare teams in Deerfield, IL.
Responsibilities
- Ensure the team is providing initial coordination of information with clients, their family members, physicians, referral sources, facility staff, agency staff, and any others involved in assuring continuity of client care
- Inform referral sources, clients, family members, and physicians about response time and plans for follow up
- Inform clinical office staff of potential admits and accurately document information in the clients medical record
- Input patient information into the EMR and update EMR with the correct level of care
- Complete and copy necessary paperwork to ensure patient files are prepared for admission
- Establish, maintain, and close patients EMR and paper charts in accordance with regulatory and office guidelines
- Verification of insurance coverage
- Collaborate as needed to procure needed information
- Create morning and evening census reports
Requirements
- Associates degree is preferred
- Equivalent experience in medical terminology and/or working in healthcare environments
- Minimum of two years experience working in hospice, home care, or other health care environment required
- Experience in dealing with a variety of payers in healthcare, e.g., Medicare, Medicaid, and commercial payors, preferred
- Customer service focused, including the ability to set the caller at ease and encourage open dialogue about the clients needs
- Ability to work with a variety of team members representing other functional areas that directly or indirectly impact client intake/admission
- Ability to maintain composure in addressing stressful and/or challenging situations
- Prior use of Electronic Medical Records (EMR), preferred
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
