Responsibilities
Ensure initial coordination of information with clients, family members, physicians, and referral sources
Inform referral sources, clients, family members, and physicians about response time and follow-up plans
Document information in the clients' medical records
Input and update patient information in the EMR
Complete necessary paperwork for patient files
Verification of insurance coverage
Create morning and evening census reports
Requirements
Associates degree is preferred or equivalent experience in medical terminology and/or healthcare environments
Minimum of two years experience in hospice, home care, or healthcare environment
Experience dealing with various healthcare payers like Medicare, Medicaid, and commercial payors preferred
Customer service focused
Ability to maintain composure in stressful situations
Prior use of Electronic Medical Records (EMR) preferred
Must be a licensed driver with an insured automobile in good working order
