About the role

  • Hospice Intake Manager at JourneyCare Hospice ensuring client information coordination with various stakeholders. Responsible for managing admissions and documentation processes in a healthcare environment.

Responsibilities

  • Ensure initial coordination of information with clients, family members, physicians, and referral sources
  • Inform referral sources, clients, family members, and physicians about response time and follow-up plans
  • Document information in the clients' medical records
  • Input and update patient information in the EMR
  • Complete necessary paperwork for patient files
  • Verification of insurance coverage
  • Create morning and evening census reports

Requirements

  • Associates degree is preferred or equivalent experience in medical terminology and/or healthcare environments
  • Minimum of two years experience in hospice, home care, or healthcare environment
  • Experience dealing with various healthcare payers like Medicare, Medicaid, and commercial payors preferred
  • Customer service focused
  • Ability to maintain composure in stressful situations
  • Prior use of Electronic Medical Records (EMR) preferred
  • Must be a licensed driver with an insured automobile in good working order
