About the role

  • Perform case management telephonically within the scope of licensure for members at risk of or with behavioral health and/or substance use disorder needs
  • Use screening criteria and clinical judgment to assess member needs
  • Conduct assessments to identify individual needs and develop care plans to address objectives and goals
  • Create, monitor and evaluate effectiveness of care plans and modify as needed
  • Support member access to appropriate, quality and cost-effective care
  • Coordinate with internal and external resources and collaborate with providers to meet member needs
  • Assist members in transitioning to higher levels of care and facilitate discharge plans from inpatient settings
  • Participate in required in-person onboarding and training sessions as needed

Requirements

  • MA/MS in social work, counseling, or a related behavioral health field OR a degree in nursing (minimum) or equivalent combination of education and experience
  • Minimum of 3 years clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment
  • Current active unrestricted license required (examples listed): RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist, able to practice within scope of licensure in applicable U.S. states/territory
  • Experience in case management and telephonic coaching with members with complex psychiatric/substance abuse and/or medical disorders preferred
  • Experience working with medical diagnosis preferred
  • CCM certification preferred
  • Must be within a reasonable commuting distance of the posting location(s) unless accommodation is granted
  • May be required to become vaccinated against COVID-19 and Influenza for certain patient/member-facing roles
