Perform case management telephonically within the scope of licensure for members at risk of or with behavioral health and/or substance use disorder needs
Use screening criteria and clinical judgment to assess member needs
Conduct assessments to identify individual needs and develop care plans to address objectives and goals
Create, monitor and evaluate effectiveness of care plans and modify as needed
Support member access to appropriate, quality and cost-effective care
Coordinate with internal and external resources and collaborate with providers to meet member needs
Assist members in transitioning to higher levels of care and facilitate discharge plans from inpatient settings
Participate in required in-person onboarding and training sessions as needed
Requirements
MA/MS in social work, counseling, or a related behavioral health field OR a degree in nursing (minimum) or equivalent combination of education and experience
Minimum of 3 years clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment
Current active unrestricted license required (examples listed): RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist, able to practice within scope of licensure in applicable U.S. states/territory
Experience in case management and telephonic coaching with members with complex psychiatric/substance abuse and/or medical disorders preferred
Experience working with medical diagnosis preferred
CCM certification preferred
Must be within a reasonable commuting distance of the posting location(s) unless accommodation is granted
