About the role

  • Reviews patient files and Registry information to make appropriate Care Plan recommendation.
  • Effectively communicates patient issues and care plan recommendations to Provider and Psychiatrist plus implement their recommendations.
  • Promotes patient engagement and robust education.
  • Provides leadership for CoCM team members to improve patient outcomes.
  • Performs psychosocial assessments using evidence-based, standardized behavioral health tools and protocols.
  • Participates in Care Team Conferences.

Requirements

  • Bachelor’s Degree (Required)
  • 2+ years of experience in behavioral healthcare management (required)
  • Strong knowledge of federal and state programs
  • Bilingual candidates encouraged to apply
