About the role
- Pflegefachkraft performing individualized care to patients at home without tour care. Supporting patients in a relaxed and compassionate environment with assistance from experienced teams.
Responsibilities
- Individualized work in a comfortable private household — not a touring schedule, only ONE patient
- Great work–life balance through structured 12-hour shifts
- Predictable time off thanks to reliable shift scheduling
- Intensive medical monitoring, basic and treatment-related nursing care in the home setting
- Point of contact for the patient and their relatives
- Coordination and communication with therapists
- Implementation of rehabilitative measures and social support
- Outings and excursions with the patient and, if applicable, relatives
- Maintain nursing documentation
Requirements
- State-certified geriatric nurse, registered nurse (adult), nursing specialist, or pediatric nurse
- Alternatively: qualified specialist for out-of-hospital intensive care
- Interest in outpatient care of patients requiring intensive nursing
- Passion for 1:1 care
- Conscientious, independent, and responsible working style
- Empathy and a caring approach to patient support
- Willingness to take part in activities with the patient
