About the role

  • Case Manager providing supportive services and case management to homeless individuals. Collaborating with various programs and ensuring adherence to policies in Lowell, MA.

Responsibilities

  • Provide case management services to participants in a program serving episodically homeless individuals with relapse histories.
  • Perform home visits & provide group facilitation.
  • Ensure adherence to policies and protocols for programs serving chronically homeless individuals.
  • Develop regular program schedule including activities, meetings, meals, etc.
  • Conduct initial service needs assessment with all clients entering the program.
  • Develop Individual Service Plan with each participant within three days of entry into the program.
  • Maintain daily contact with clients actively involved in the program.
  • Maintain appropriate files and case notes for each participant.
  • Provide crisis intervention (24-hour cell phone coverage).

Requirements

  • Bachelor’s Degree in related field.
  • May be substituted with a minimum of 5 years working with the target population.
  • Substance abuse experience required.
  • Excellent interpersonal and organizational skills.
  • Previous experience with group facilitation preferred.
