About the role

  • Provides mental health assessment, diagnosis, and crisis intervention for adult and child patients. Coordinates care and assists in accessing community resources while ensuring quality management activities.

Responsibilities

  • Provides mental health assessment, diagnosis, treatment, and crisis intervention services
  • Coordinates care for patients and assists them in accessing services and resources
  • Oversees all utilization management activity to include authorization of services
  • Ensures all patient discharge planning documentation is clear and timely
  • Conducts clinical assessments for interpretation and treatment intervention
  • Provides case management and care coordination services for clients/patients
  • Acts as a liaison between patients, referral sources, and funders
  • Participates in quality improvement activities for the department or program

Requirements

  • Master’s Degree Social Work, Counseling, or related field
  • Ability to remain patient and calm in stressful situations
  • Knowledge of community resources and referrals
  • Understanding of psychosocial/medical needs of clients
  • LCSW - Licensed Clinical Social Worker - State of Pennsylvania
  • LPC - Licensed Professional Counselor - State of Pennsylvania
  • LSW - Licensed Social Worker - State of Pennsylvania
