About the role

  • Community Care Case Manager responsible for enhancing patient care and collaborating with healthcare providers. Engaging with members to address comprehensive health needs and optimize wellness.

Responsibilities

  • Help elevate patient care to new levels as part of the Community Care team
  • Collaborate with members, providers, and community organizations to address health care and social determinant needs
  • Facilitate the delivery of appropriate benefits and healthcare information
  • Develop, implement, and support Health Strategies, tactics, policies, and programs
  • Act as a liaison with member/client /family, employer, provider(s), insurance companies, and healthcare personnel as appropriate
  • Interact with members/clients telephonically or in person, including potential home visits
  • Assess and analyze member/client needs and develop a plan of care
  • Prepare documentation of case work activities as appropriate
  • Consult with interdisciplinary teams to help maximize health outcomes
  • Provide educational and prevention information

Requirements

  • Candidate must have active and unrestricted Connecticut (CT) Registered Nurse (RN) License
  • Candidate must be willing to travel 55-75% within designated areas (Yale New Haven)
  • 3+ years clinical practical experience preference: (diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac) with Medicare members
  • 2+ years case management, discharge planning, and/or home health care coordination experience
  • Excellent analytical and problem-solving skills
  • Effective communications, organizational, and interpersonal skills
  • Ability to work independently (may require working from home)
  • Proficiency with standard corporate software applications, including MS Word, Excel, Outlook and PowerPoint, as well as some special proprietary applications
  • Efficient and effective computer skills including navigating multiple systems and keyboarding
  • Certified Case Manager National professional certification (CRC, CDMS, CRRN, COHN, or CCM)
