Community Care Case Manager responsible for enhancing patient care and collaborating with healthcare providers. Engaging with members to address comprehensive health needs and optimize wellness.
Responsibilities
Help elevate patient care to new levels as part of the Community Care team
Collaborate with members, providers, and community organizations to address health care and social determinant needs
Facilitate the delivery of appropriate benefits and healthcare information
Develop, implement, and support Health Strategies, tactics, policies, and programs
Act as a liaison with member/client /family, employer, provider(s), insurance companies, and healthcare personnel as appropriate
Interact with members/clients telephonically or in person, including potential home visits
Assess and analyze member/client needs and develop a plan of care
Prepare documentation of case work activities as appropriate
Consult with interdisciplinary teams to help maximize health outcomes
Provide educational and prevention information
Requirements
Candidate must have active and unrestricted Connecticut (CT) Registered Nurse (RN) License
Candidate must be willing to travel 55-75% within designated areas (Yale New Haven)
3+ years clinical practical experience preference: (diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac) with Medicare members
2+ years case management, discharge planning, and/or home health care coordination experience
Excellent analytical and problem-solving skills
Effective communications, organizational, and interpersonal skills
Ability to work independently (may require working from home)
Proficiency with standard corporate software applications, including MS Word, Excel, Outlook and PowerPoint, as well as some special proprietary applications
Efficient and effective computer skills including navigating multiple systems and keyboarding
Certified Case Manager National professional certification (CRC, CDMS, CRRN, COHN, or CCM)
