About the role
- Targeted Case Manager at Addiction Recovery Care assisting individuals in gaining access to recovery supports and services. Aiding in assessment, coordination, and documentation to ensure client success.
Responsibilities
- Assist individuals eligible under the state plan in gaining access to needed recovery supports, medical, social, educational, and other services
- Assess the individualized needs of an individual
- Coordinate provision of services
- Document services and outcomes
- Maintain confidentiality and comply with company, state, federal and HIPAA rules and regulations
- Chart for insurance purposes
- Maintain a positive, professional attitude toward residents, staff and volunteers
- Create a safe and healthy environment for clients
- Have a sincere commitment to client’s recovery
- Transport clients to appointments when necessary
- Monitor client's emotional well-being and behavior
- Connect client with family and community resources
- Connect client with legal entities when necessary
- Motivate clients toward goal achievement
- Meet with clients weekly
- Help residents with long-term care plan development and educational resources
Requirements
- Bachelor’s degree in social work or human services field required
- 1 year post grad work experience or Master's Degree required
- Must have a current driver’s license
- Basic computer skills
- Ability to complete and submit reports
- Be self-motivated
- Excellent documentation skills
- Excellent written and verbal communication skills
- Strong working knowledge of computer skills and use of search engines
- Ability to handle crisis situations in a calm and supportive manner
- Maintain a positive, professional attitude toward clients and staff
