Lead Advocate assisting with behavioral health programming to support patient population at Benchmark Health. Managing patient panels and coordinating care with providers and resources.
Responsibilities
Screen and assess patient for behavioral health symptoms and conditions
Work with patients to set measurable personalized goals and provide brief behavioral interventions
Participate in regularly scheduled caseload consultation with the psychiatric consultant
Serve as ongoing liaison between providers to coordinate patient’s care
Systematically track treatment response, any changes in clinical symptoms or treatment side effects that may arise
Facilitate treatment plan changes for patients who are not improving as expected
Collaborate with and provide clinical support to non-licensed Advocates and Behavioral Health Care Managers
Coordinate care with referral sources as clinically indicated
Offer patients continuous follow up and redirection to new resources as needed
Partner with team on process improvement efforts
Requirements
Master’s in Social Work, Mental Health Counseling, or Marriage and Family Therapy
LICSW, LMHC, LADC-1, or LMFT in the state of practice required
Minimum of 3-5 years of experience of direct clinical practice in behavioral health
Previous experience providing measurement-based care preferred
Previous experience managing clinical escalations and performing safety risk assessments
Previous experience working with patients with serious mental illness and substance use disorder preferred
Comfortability and experience working with children and adolescents preferred
Previous experience with AIMS Collaborative Care Model strongly preferred
