About the role

  • Lead Advocate assisting with behavioral health programming to support patient population at Benchmark Health. Managing patient panels and coordinating care with providers and resources.

Responsibilities

  • Screen and assess patient for behavioral health symptoms and conditions
  • Work with patients to set measurable personalized goals and provide brief behavioral interventions
  • Participate in regularly scheduled caseload consultation with the psychiatric consultant
  • Serve as ongoing liaison between providers to coordinate patient’s care
  • Systematically track treatment response, any changes in clinical symptoms or treatment side effects that may arise
  • Facilitate treatment plan changes for patients who are not improving as expected
  • Collaborate with and provide clinical support to non-licensed Advocates and Behavioral Health Care Managers
  • Coordinate care with referral sources as clinically indicated
  • Offer patients continuous follow up and redirection to new resources as needed
  • Partner with team on process improvement efforts

Requirements

  • Master’s in Social Work, Mental Health Counseling, or Marriage and Family Therapy
  • LICSW, LMHC, LADC-1, or LMFT in the state of practice required
  • Minimum of 3-5 years of experience of direct clinical practice in behavioral health
  • Previous experience providing measurement-based care preferred
  • Previous experience managing clinical escalations and performing safety risk assessments
  • Previous experience working with patients with serious mental illness and substance use disorder preferred
  • Comfortability and experience working with children and adolescents preferred
  • Previous experience with AIMS Collaborative Care Model strongly preferred
