About the role

  • Be accountable for the community outreach account plan for assigned geography which may include one or multiple markets
  • Conduct thorough research on community provider practices, organizational structure to drive account plans
  • Conduct thorough research to understand patient's healthcare needs, healthcare utilization and best number to call
  • Outreach and conduct in-person visits to physician offices, hospitals and skilled nursing facilities to educate community providers and engage patients into the HarmonyCares program
  • Collect, analyze and report patient outcomes and experiences to community physicians
  • Conduct telephonic outreach to eligible patients or caregivers to enroll patients in the HarmonyCares program to support team engagement goals
  • Understand the program in depth, explain it to patients, caregivers and providers and handle objections creatively
  • Be the face of HarmonyCares in the community and collaborate closely with physician leaders to support community relationships
  • Leverage electronic tools, spreadsheets and reports to facilitate research, identify innovative approaches, and document and present plans and progress
  • Meet or exceed established monthly engagement targets

Requirements

  • College degree or 5 years of progressive career growth
  • 3+ years of related experience in sales, account management, community outreach or patient engagement in a medical/healthcare industry
  • An understanding of value-based care and how it can impact patient outcomes
  • Proficiency in Microsoft Excel, Word, and PowerPoint, ability to generate Excel reports
  • A dynamic and engaging personality with excellent interpersonal, verbal and written communication skills
  • Detail oriented, with the ability to think on your feet, responsive with great follow-up
  • Compassion, an interest in people and a professional presentation
  • Curious and a continuous learner, a passion for solving problems with a get-it-done attitude
  • Bilingual in Spanish, Mandarin or other language a big plus (preferred)
