About the role

  • Behavioral Health Case Manager I responsible for performing case management telephonically and through home visits. Supports members with behavioral health and substance abuse needs within an established care plan helping coordinate with internal and external resources.

Responsibilities

  • Performs case management telephonically and/or by home visits
  • Uses appropriate screening criteria and clinical judgment to assess member needs
  • Conducts assessments to identify individual needs and develops care plans
  • Monitors and evaluates effectiveness of care plans and modifies as needed
  • Supports member access to appropriate quality and cost effective care
  • Coordinates with internal and external resources to meet member needs and collaborates with providers

Requirements

  • Requires MA/MS in Social Work, Counseling, or a related Behavioral Health field or a degree in nursing
  • Minimum of 3 years clinical experience in social work counseling
  • Experience with complex psychiatric and substance abuse disorder treatment
  • Current, active, unrestricted license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW or Clinical Psychologist
  • Experience in case management
  • Telephonic and/or in person coaching
  • Experience working on multiple computer platforms
  • Excellent typing skills
  • Bilingual communication skills a plus
