Responsibilities
Responsible for the provision of medical social work services to patients receiving care in the hospital setting.
Assess the social, psychological, cultural, environmental, and financial situations of each referred patient.
Collaborate with patients, families, healthcare team members, and community organizations.
Develop and evaluate effective transition plans considering the client's diagnosis, prognosis, and available health care benefits.
Functions in one of the following practice settings: Hospitals Only.
Requirements
Experience: 18 months of recent acute care RN experience.
Facility supported: Members of the team are required to work assigned shifts in all units within the case management specialty.
Required to work in 4 hospitals in the Southside or Western Tidewater regions.
Must enter at least 48 hours of prescheduled time per calendar month in API.
Must include availability to be scheduled on a weekend.
