About the role

  • Case Manager providing specialized care for women escaping human trafficking and sexual exploitation. Involves case management, service coordination, and support towards independence and well-being.

Responsibilities

  • Help clients gain access to needed medical, social, educational, and other services
  • Conduct assessments of eligible clients to determine service needs
  • Develop individualized integrated care plans
  • Coordinate all services for each individual client, including advocacy and information about community resources
  • Document services, including identifying resources, writing progress notes, and submitting as required

Requirements

  • BA degree in human service field, preferably Social Work
  • Case Management certification in proper jurisdiction required
  • 2 years experience in providing services to victims of crime including trafficking and sexual assault
  • Must have a valid state driver’s license
