About the role

  • CAP Case Manager supporting the Community Alternatives Program for Disabled Adults and Children at Healthkeeperz. Conducting assessments, care planning, and service coordination with individuals in their homes.

Responsibilities

  • Conduct assessments and determine CAP eligibility
  • Develop individualized care plans based on identified needs
  • Coordinate services with families, providers, and community agencies
  • Perform home visits and follow-up monitoring
  • Maintain accurate documentation in E-CAP; review aide notes; support compliant billing
  • Ensure compliance with CAP and state program guidelines
  • Participate in required training and continuing education

Requirements

  • Bachelor’s in Social Work + 1 year related experience
  • Bachelor’s in Human Services + 2 years related experience
  • Bachelor’s in Non-Human Services + 2 years related experience
  • Current NC RN license + 1 year case management experience
  • Apprentice/Trainee (Bachelor’s or RN without full experience) – requires ongoing supervision
  • Knowledge of casework, care coordination, and social services practices
  • Familiarity with community resources
  • Strong communication, time management, and organization
  • Ability to travel for visits and meetings
  • Proficiency with electronic documentation and virtual communication tools
