Responsibilities
- coordinating referrals, education, and interdisciplinary support for hospice and palliative care services
- ensuring that members with serious or terminal illnesses receive appropriate care
- supporting accurate member understanding of care goals
- facilitating engagement with relevant community resources
- conducting serious illness conversations with members and caregivers
- educating members, caregivers, and clinical staff on hospice and palliative care options and benefits
- coordinating ongoing care and communication between CareBridge and community palliative organizations
- tracking referrals and developing strategies for process improvements in member engagement and service delivery
- maintaining supportive relationships with both internal and external stakeholders to ensure continuity of care
- developing and implementing educational materials and training programs for internal teams
- assisting with process design and improvement efforts unique to the palliative care navigator function
Requirements
- Requires a bachelor’s degree in a related field, such as BSN
- minimum of 3 years of clinical experience in a hospice or palliative care organization
- RN certification, APC, or IFOC certification preferred
- MSW, LCSW, or MDiv preferred
