About the role

  • LTSS Service Coordinator conducting service coordination functions for a caseload in Lee/Sarasota County. Requires fieldwork and experience with social work agency, preferable bilingual in Spanish.

Responsibilities

  • Conduct service coordination functions for a defined caseload of individuals
  • Facilitate the Person Centered Planning process
  • Conduct face-to-face program assessments
  • Apply motivational interviewing techniques for evaluations
  • Coordinate and manage an individual’s waiver
  • Manage non-clinical needs of members
  • Document short and long-term service and support goals
  • Identify members that would benefit from alternative service levels
  • Submit utilization/authorization requests

Requirements

  • Requires BA/BS degree
  • Minimum of 2 years of experience working with a social work agency
  • Bilingual in English/Spanish strongly preferred
  • BA/BS degree in health care related field preferred
  • Long-term care experience preferred
