About the role

Responsibilities
- Coordinate and oversee all direct care patient services provided by clinical personnel
- Develop and implement clinical operations for a specific location
- Manage assignment of caregivers and oversee patient care delivery
- Conduct assessments and updates of care plans for patients
- Ensure compliance with regulatory agencies and billing requirements
- Supervise clinical employees and participate in hiring and training of staff
- Analyze situations and evaluate alternatives for performance improvement
- Provide direct patient care in emergencies and act as Branch Director in their absence
Requirements
- Associate degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) strongly preferred
- Active and unrestricted Registered Nurse (RN) license or Compact State RN license in South Carolina
- Proof of current CPR card and/or BLS certification
- Valid driver’s license and reliable transportation
- Minimum of 2 years of experience as a Registered Nurse (RN)
- Background in home-based care settings, including traditional home health, hospice, or palliative care is strongly preferred
- Management and people leadership experience is required
- OASIS experience is preferred
- OASIS certification (HCS-O, COQS, and/or COS-C), preferred
- Homecare Homebase (HCHB) experience is preferred
- Foundational knowledge and basic understanding of CMS PDGM is preferred
