About the role

  • SMI Care Manager coordinating care for individuals with Serious Mental Illness at Independent Living Systems. Ensuring comprehensive, person-centered support through collaboration with various teams.

Responsibilities

  • Conduct comprehensive assessments of members’ mental health status, social needs, and functional abilities to inform individualized care planning.
  • Develop, implement, and regularly update personalized care plans in collaboration with members, families, and healthcare professionals.
  • Coordinate access to medical, psychiatric, and social services, ensuring timely referrals and follow-up.
  • Monitor member progress through regular contact, adjusting care plans as needed to address changing needs or challenges.
  • Advocate for members’ rights and needs within healthcare systems and community resources to promote equitable access to services.

Requirements

  • Bachelor’s degree in Social Work, Nursing, Psychology, or a related health care field
  • At least two years of experience working with individuals diagnosed with Serious Mental Illness (SMI)
  • Knowledge of mental health care systems, community resources, and relevant regulatory requirements
  • Ability to maintain confidentiality and adhere to ethical standards in member care
  • Relevant experience may substitute for the educational requirement on a year-for-year basis
