About the role

Responsibilities
- Conduct comprehensive assessments of members’ mental health status, social needs, and functional abilities to inform individualized care planning.
- Develop, implement, and regularly update personalized care plans in collaboration with members, families, and healthcare professionals.
- Coordinate access to medical, psychiatric, and social services, ensuring timely referrals and follow-up.
- Monitor member progress through regular contact, adjusting care plans as needed to address changing needs or challenges.
- Advocate for members’ rights and needs within healthcare systems and community resources to promote equitable access to services.
Requirements
- Bachelor’s degree in Social Work, Nursing, Psychology, or a related health care field
- At least two years of experience working with individuals diagnosed with Serious Mental Illness (SMI)
- Knowledge of mental health care systems, community resources, and relevant regulatory requirements
- Ability to maintain confidentiality and adhere to ethical standards in member care
- Relevant experience may substitute for the educational requirement on a year-for-year basis
