- Manage a panel of rising and high intensity members for chronic disease and behavioral health care
- Collaborate with members to create and oversee care plans
- Conduct assessments and screeners to identify behavioral health needs
- Participate in case conferences and develop care plans in collaboration with members
- Conduct clinical visits and follow-ups to monitor therapeutic interventions
- Perform medication reconciliation and compliance education
- Utilize health records and scheduling platforms to document interactions and track tasks

- Graduate of an accredited school of nursing (R.N.)
