About the role

  • Assist Primary Care Physician, Nurse Practitioners and Clinical Teams in delivery of primary health care and patient care management in residential settings
  • Support Patient Care through clinical partnership with Nurse Practitioners, Physicians, and Care Management Teams
  • Coordinate daily travel itinerary and transport clinicians to scheduled appointments
  • Prepare patients for examination, treatment, and minor procedures when needed
  • Secure patient consent forms and record patient care documentation accurately and timely
  • Coordinate patient care as directed by Clinicians and company policies
  • Perform phlebotomy (venipuncture only), EKGs, and label/store/package specimens for lab delivery
  • Assist in identifying and closing quality gaps; document and update patient care plans and SDoH assessments in EMR
  • Maintain compliance with CLIA, OSHA, and HIPAA guidelines
  • Prepare and maintain supplies and equipment; manage medical bag and company vehicle maintenance
  • Attend required meetings and in-services; interact with patients and families

Requirements

  • High School graduate or GED equivalent
  • Graduate of an accredited Medical Assistant, Phlebotomy, EMT, related medical education/training program OR 1 year of experience serving as a Medical Assistant or Phlebotomist
  • Current CPR certification that meets Company guidelines
  • Knowledge of medical terminology
  • Basic computer literacy, including EMR, iPad, and GPS experience
  • Strong interpersonal communication and customer service skills
  • Must maintain a valid driver’s license and good driving record
  • Required to maintain compliance with CLIA, OSHA, and HIPPAA guidelines
  • Phlebotomy (venipuncture) and EKG skills as requested by Clinician
  • Ability to prepare and maintain supplies, medical bag, and company vehicle
  • Preferred: Certified Medical Assistant
  • Preferred: Six months of experience in patient care, preferably geriatric care
  • Preferred: Strong Phlebotomy skills
  • Preferred: Desire and ability to obtain MA certification within 180 days of hire
  • Physical requirements: full range body motion including handling/lifting patients; ability to drive up to 150 miles daily; ability to lift/carry up to 50 lb; ability to read and understand road maps; sit or stand for long periods of time
