Recommend and assist with access to medical, educational, social, vocational, housing, and more to support program goals.
Initiate referrals to the next level of care, and coordinate with physical health providers to ensure whole-person care, and advocate in the best interest of clients.
Provide services for individual counseling and group sessions for individuals that are justice involved with Substance Use Disorder and Mental Health Disorders.
Collaborate with Program Manager in client care for assessments, groups, individual sessions, along with the treatment planning.
Develop, evaluate, and implement treatment goals and plans in conjunction with the FPP statement of work.
Provide individual supportive coaching in all areas of clients’ life.
Perform psychosocial and risk assessments and maintains a daily log of client contacts.
Provide support to all clients including on-call crisis intervention covering nighttime and weekend hours.
Conduct and oversee psychoeducational groups as directed by Program Manager.
Perform shift management duties in coordination with other staff as directed by Program Manager.
Function as a liaison between client, program, and federal Probation Officer.
Comply with all charting and documentation requirements per contract, funding agency, and TURN policy.
Prepare critical care issues for discussion during supervision and Daily Organizational Staff Meetings.
Collect necessary client demographic data and enters it on the TURN Treatment Team Services Data Collections Form.
Provide information about the program to community agencies connected to the mental and substance abuse services system.
Comply with all TURN and program rules, policies and procedures including HIPAA standards.
Requirements
Master’s degree in a behavioral science and a background of experience providing clinical services to individuals with Substance Use Disorders and CA BBS registration or license (MFT, LCSW, LPCC) required.
Valid CA driver’s license and current auto insurance per contract requirements required.
Experience providing mental health and/or substance use disorder services preferred.
Cultural competency understanding required.
Bilingual preferred.
Ability to problem solve and react appropriately to emergency situations.
Ability to work independently and effectively collaborate with a team.
Knowledge of Microsoft Office and EHR software preferred.
