Responsibilities
Deliver virtual primary and preventive care to underserved patients across Texas
Manage chronic conditions, acute concerns, and routine follow-ups through telemedicine
Build trusted relationships with patients who have limited access to consistent medical care
Support and refine clinical programs focused on hypertension, diabetes, medication adherence, and reducing preventable hospitalizations
Partner with care coordinators, CHWs, and RNs to close care gaps and ensure seamless follow-up
Provide clinical guidance during case reviews and escalations
Collaborate with product and operations teams to improve care delivery workflows

Requirements
2+ years of clinical experience, preferably serving medically or socially complex populations
Active Texas NP license
Bilingual - Spanish and English fluency required
Must be physically located in Travis County (Austin,TX) with access to a quiet, HIPAA-compliant, internet-connected workspace
Experience supporting patients with chronic disease, behavioral health needs, SUD, homelessness, or Medicaid coverage
Comfort working within a multidisciplinary care model
Board certification (or eligibility) in Family Medicine
Comfortable delivering telemedicine and working across multiple digital platforms
Ability to care for patients across the lifespan (pediatrics to geriatrics)
Familiarity with FQHCs, safety-net care, or Medicaid populations preferred
Startup or fast-paced environment experience is a plus
