About the role

  • Case Manager RN providing nursing care for home health patients in Washington's Olympic Peninsula. Supervising LPNs and HHAs while promoting continuity of care in various settings.

Responsibilities

  • Assesses, plans, implements, coordinates and evaluates the nursing care of patients in their place of residence
  • Promotes continuity of care for patients on their assigned caseload
  • Provides direct patient care and supervises the care provided by LPNs and HHAs
  • Works in collaboration with all home health disciplines assigned to provide care for patients on caseload
  • Applies principles of learning in teaching patient’s self-management and methods to regain independence
  • Provides on-call support as assigned for patients

Requirements

  • Associate’s degree required
  • B.S.N. preferred
  • Minimum of one year recent experience preferred in home health, acute or long-term care
  • Current Washington State Registered Nurse license, required
  • Current BLS certification, required
  • Valid Washington driver’s license, required
  • Proof of insurance on automobile used for work, required
  • Must have computer internet connectivity (OMHH work laptop provided)
  • Must have reliable automobile transportation for entire scheduled shift to travel to patient residences
