- Provide virtual support to individuals and families affected by suicide ideation and attempts
- Develop and coordinate community resources to address medical, behavioral, violence prevention, and social service needs
- Assess risk, perform crisis interventions, and ensure safety through tailored safety plans
- Collaborate with members to assess care options and develop and update service plans
- Educate members about mental and physical health, abuse prevention, medication, and available resources
- Assist in scheduling appointments and follow-ups, ensuring members attend their sessions and appointments
- Maintain accurate and up-to-date records of all case management activities
Requirements
- Bachelor’s degree in psychology, social work, or another applicable behavioral health discipline from an accredited institution
- Master’s degree in a related field (preferred)
- At least 2 years of experience in a related role, preferably working with at-risk adolescents and young adults
- Crisis intervention training or similar certification is required
- Knowledge of local resources, social determinants of health, mental health, substance abuse disorders, interpersonal violence, and social issues
