- Maintain detailed documentation and case records in compliance with regulatory standards and organizational policies.
Requirements
- Bachelor’s degree in Social Work, Human Services, Nursing, or a related field
- Experience in case management within healthcare or social services settings
- Knowledge of Medicaid policies and procedures
- Relevant experience may substitute for the educational requirement on a year-for-year basis.
- Master’s degree in Social Work (MSW) or related discipline (preferred)
- Certification in Case Management (CCM) or equivalent (preferred)
- Experience working with diverse populations in community-based settings (preferred)
- Familiarity with electronic health records (EHR) and care coordination software (preferred)
- Training in trauma-informed care or behavioral health interventions (preferred)
