About the role

  • LTSS Service Coordinator conducting service coordination functions for individuals in Broward County. Facilitating Person Centered Planning processes and collaborating with interdisciplinary care teams.

Responsibilities

  • Responsible for conducting service coordination functions for a defined caseload of individuals in specialized programs
  • Facilitate the Person Centered Planning process that documents the member’s preferences, needs and self-identified goals
  • Conduct assessments and develop a comprehensive Person Centered Support Plan (PCSP)
  • Interface with Medical Directors and participate in interdisciplinary care rounds
  • Engage the member's circle of support and overall management of physical/behavioral health/LTSS needs
  • Manage non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities
  • Submits utilization/authorization requests to utilization management

Requirements

  • Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency
  • Bilingual in English/Spanish strongly preferred
  • BA/BS degree in health care related field preferred
  • Long-term care experience preferred
