About the role

  • LTSS Service Coordinator responsible for conducting assessments, developing support plans, and coordinating care for a defined caseload in Florida. Requires a BA/BS degree and bilingual skills preferred.

Responsibilities

  • Conduct service coordination functions for a defined caseload of individuals in specialized programs
  • Facilitate the Person Centered Planning process
  • Conduct assessments and develop comprehensive Person Centered Support Plans (PCSP)
  • Engage with Medical Directors and participate in interdisciplinary care rounds
  • Manage members' physical health (PH)/behavioral health (BH)/LTSS needs
  • Perform face to face program assessments
  • Identify members with potential clinical health care needs
  • Coordinate member’s cases with clinical healthcare management

Requirements

  • Requires BA/BS degree
  • Minimum of 2 years of experience working with a social work agency
  • Bilingual in English/Spanish strongly preferred
  • LTSS, case management, social work or hospital discharge planning experience preferred
