About the role

  • Social Worker assessing 0-36 month applicants for Early Start program services. Collaborating with families and coordinating eligibility assessments for developmental services.

Responsibilities

  • Lead the assessment of applicants (0-36 months) applying for services under the Early Start program
  • Facilitate IDCT eligibility assessment coordination and develop Initial IFSP within 45 days of referral
  • Contact the client/family/guardian to make appointments
  • Conduct interviews to obtain social, emotional, developmental and familial information for home-based assessments
  • Clarify requests for services and client/family needs
  • Design, monitor and revise Individual Family Service Plan (IFSP)
  • Work with client families to assess needs, preferences and life choices
  • Confirm immediate needs and provide necessary interventions or services
  • Complete Early Start Assessment report
  • Maintain applicant records, Case and Title XIX Progress notes
  • Participate in staff and community meetings and task forces as needed

Requirements

  • Master’s Degree in Social Work, Early Childhood Education or related human services field preferred; Bachelor’s degree in social work with 1 year experience working with 0-36 month population required or related field required.
  • 2+ years (post-doctoral) work with developmentally disabled population
  • Fluency in Spanish to perform the outlined responsibilities of the position
  • Thorough knowledge of colloquial and relevant technical/functional terminology
  • Ability to speak clearly and be understood by native speakers in Spanish
  • Ability to understand relevant spoken/written dialects in Spanish, as appropriate
  • Interviewing techniques to ensure person-centered thinking
  • Knowledge of counseling principles, objectives and techniques
  • Knowledge of normal and abnormal individual and group behavior, human growth and development of family relationships
  • Ability to maintain case records in a clear and concise manner
  • Written communication skills: ability to develop standard business communications
  • Oral communication skills: ability to convey complex information clearly
  • Ability to operate basic office equipment
  • Basic familiarity with MS Word®, MS Excel®, MS PowerPoint®, MS Outlook® or similar software applications
  • Facilitate discussions and collaboration
  • Build partnerships and trust through active listening and openness to new ideas
