About the role

  • Hospice Account Executive expanding care in Carbon, Luzerne, & Monroe Counties to deliver compassionate care. Liaising with referral sources to ensure high-quality service and patient satisfaction.

Responsibilities

  • Prepares and presents presentations of various in-services to referral sources and follows-up with the referral sources.
  • Educates referral sources on the components of Home Health services.
  • Serves as a liaison between Home Health and physicians, hospitals, long-term care facilities, assisted living facilities and all other referral sources.
  • Communicates frequently with each referral source to ensure that the expectations of the referral source and the needs of the patients are being met.
  • Assists in identifying and resolving any issue, dissatisfaction, or problem that the referral source is experiencing with quality of care.
  • Oversees and monitors primary relationships for consistency and quality.
  • Submits, in a timely manner, written documentation of all contacts, referrals, and contracts.
  • Assists in the development of new programs to meet the needs of the community.
  • Establishes relationships with referral sources such as physicians, hospitals, long-term care facilities and assisted living facilities.

Requirements

  • 3 - 5 Years Hospice / Home Health Experience
  • Proven success in reaching/exceeding quotas
  • Excellent organizational, communication and interpersonal skills
  • Strategic thinker who can help evolve marketing calendars and processes
  • Self-starter - strong sense of urgency and initiative
  • Ability to multitask in a fast-paced environment
  • Work well with multiple teams and partners
  • Solid computer skills (Excel, Word, PowerPoint, etc.)
  • Positive collaborator; balances taking initiative and direction, as needed
  • Ability to work collaboratively with multiple cross-functional teams: sales, marketing, operations, etc.
