About the role

  • LTSS Service Coordinator managing specialized programs for individuals with diverse care needs. Conducting assessments and collaborating with interdisciplinary teams to develop care plans in Florida.

Responsibilities

  • Conduct service coordination functions for a defined caseload
  • Facilitate the Person Centered Planning process that documents the member’s preferences, needs and self-identified goals
  • Conduct assessments and develop a comprehensive Person Centered Support Plan (PCSP)
  • Interface with Medical Directors
  • Participate in interdisciplinary care rounds
  • Manage individuals' physical health (PH)/behavioral health (BH)/LTSS needs
  • Document short and long-term service and support goals in collaboration with the member’s chosen care team
  • Submit utilization/authorization requests to utilization management
  • Report critical incidents to appropriate internal and external parties
  • Assist in appeal or fair hearings, member grievances, appeals, and state audits

Requirements

  • BA/BS degree
  • Minimum of 2 years of experience working with a social work agency
  • Specific education, years, and type of experience may be required based upon state law and contract requirements
  • Bilingual in English/Spanish strongly preferred
  • LTSS, case management, social work or hospital discharge planning experience preferred
