About the role
- Care Manager connecting with patients for resource fulfillment in addiction treatment. Collaborating with community partners to enhance patient care and promote awareness of Crossroads.
Responsibilities
- Connect with patients monthly to address any needs and provide resources
- Connect and maintain relationships with community partners for patient resources
- Work closely with a dedicated team to provide exceptional patient care
- Participate in community awareness/engagement events to promote Crossroads and our mission
Requirements
- At minimum, High School Diploma or GED required
- Experience in a fast-paced environment, servicing many clients, is preferred
- Experience in healthcare, addiction medicine, community health, care coordination preferred
- Experience with building individualized care plans is a plus
- Knowledge of compliant documentation practices
- Lived Addiction experience is a plus
