About the role

  • Work with a caseload of 10-20 single and family households in supportive housing
  • Support the housing first model using a trauma informed care approach to maximize client success and long-term self-sufficiency
  • Conduct individual needs assessments and use outcomes to determine supports needed to maintain stability and achieve goals
  • Develop Individualized Service Plans and assist clients in planning for discharge/graduation
  • Assist clients in securing appropriate services and coordinate delivery of services (employment, housing, finance, mental health, disability resources, etc.)
  • Provide support to clients experiencing crises and implement emergency procedures
  • Create and maintain financial budgets and money management education with clients; ensure rent and utilities are paid timely
  • Maintain complete client records in accordance with agency standards and timely documentation in client file and HMIS
  • Participate in on-call rotation, staff meetings, and trainings as directed by supervisor
  • Function as a contributing member of a multidisciplinary team with Shelter House Inc. and partner agencies
  • Safely utilize Shelter House owned vehicles to drive clients to various locations

Requirements

  • A bachelor's degree in human services/ related field or commensurate experience
  • 1+ years of case management/ direct services/other related experience
  • Strong written and oral communication skills
  • Ability to prioritize competing priorities and make sound judgements
  • Ability to complete tasks while navigating frequent interruptions
  • Ability to deliver creative, resourceful solutions to unique challenges
  • Strong organizational and time management skills
  • Valid Driver’s License and reliable transportation
  • Ability to Drive a 15 passenger van
  • Willingness to work evenings and weekends when needed
  • Annual TB Test is required
  • Ability to sit or stand for long periods
  • Ability to lift items weighing 10-20 pounds
