About the role

  • Provide strength-based advocacy and case management services for 15 - 20 long-term homeless adults in site-based and scattered site housing
  • Assist participants with the housing move-in process – application, interview, and access to furniture
  • Meet weekly with residents to establish an individual service plan and remove housing success barriers
  • Inform residents on landlord expectations and tenant rights; mediate and advocate for tenant/landlord communication and conflict resolution
  • Assist with crisis prevention/intervention to ensure housing stability and promote harm reduction education
  • Arrange and provide residents’ transportation and access to medical, health, and social services; assist with forms to access benefits
  • Support residents with accessing and managing budgets, personal funds, laundry, living skills, nutrition, and rental payments
  • Onboard and provide residents with access to Housing Stabilization Services, including completing a Person Centered Plan
  • Work cooperatively with internal and external stakeholders such as Hennepin County and outreach organizations
  • Formulate strong communicative and professional relationships with landlords
  • Assist PSH team with housing intake, housing support, and Our Saviour’s Housing agreements
  • Enter detailed case notes, complete CaseFlow tasks, manage client budgets, submit expense receipts, and meet reporting deadlines
  • Prepare and file reports required by Hennepin County and OSH housing reports
  • Attend regular case management meetings, PSH office hours, trainings, and complete required Hennepin County training
  • Help train and onboard new staff and actively participate in building a culture of belonging within OSCS

Requirements

  • Demonstrated cultural fluency and commitment to diversity
  • Ability to demonstrate empathy and sympathy for others and use soft skills
  • Familiarity with and comfortable using Harm Reduction and Housing First principles
  • Able to communicate effectively in writing, by phone, and in person
  • Strong attention to detail and time management skills
  • Computer skills: Can use Google suite, MS suite, Emails, IMS, and other tech programs
  • Ability and willingness to learn about homelessness and factors impacting our participants
  • Ability to move around, drive to client locations, work on a desk and in front of a computer, sit for long periods, use keyboard and mouse, and perform repetitive motions
  • Ability to lift up to 50 pounds
  • Ability to speak, read, write, and understand people using one or more of Spanish, Somali, Arabic, Ukrainian, Russian, and Farsi/Persian (preferred but not required)
  • Relevant personal, education, or work experience equivalent to 2+ years
  • Demonstrate and maintain a good driving record, a reliable vehicle and insurance, and a valid driver’s license
  • Ability to pass a criminal background check and pass and maintain an MN Department of Human Services background study
  • Familiarity or trained in Homeless Management Information System (HMIS) and case note Software
  • Background and experience in non-profit organizations, human services, and similar programs
  • Ability to work Monday-Friday between 8:00 am and 5:00 pm
