- Provide regular updates to the care team regarding patient needs, barriers, and progress.
Requirements
- Bachelor’s Degree (preferred), Associate’s Degree (required)
- 2+ years of experience in housing support services (required)
- Strong knowledge of federal and state programs, including disability benefits, food assistance, and IHSS
- Bilingual candidates encouraged to apply
