About the role

  • Coordinate and participate in home visits and care conferences
  • Collaborate with RNCM to coordinate acute and primary care services
  • Conduct in-person comprehensive assessments of members
  • Develop and monitor member-centered plans
  • Implement risk mitigation strategies to promote health and independence
  • Maintain member records as required by DHS contract

Requirements

  • Certified Social Worker in the State of Wisconsin
  • Minimum of one (1) year experience with family care target populations
  • Four-year bachelor’s degree or more in Human Services or related field
  • Working knowledge of computers, programs, typing, and data entry
  • Ability to access members’ homes not compliant with ADA regulations
  • Ability to multi-task in a fast-paced environment
  • Current driver’s license and insurance
