About the role

  • Develops, assesses, and facilitates complex care management activities for primarily physical needs members
  • Evaluates member needs, barriers to care, and social determinants of health; focuses on member priorities
  • Develops ongoing care plans/service plans and collaborates with providers to identify specialists and community resources
  • Identifies problems/barriers to care and provides appropriate care management interventions
  • Coordinates between members/families/caregivers and the care provider team to ensure person-centered care
  • Provides ongoing follow up and monitoring of member status, condition changes, and progress toward care plan goals
  • Provides resource support for local services (employment, housing, independent living, justice, foster care) as appropriate
  • Facilitates care management and collaborates with providers/specialists to ensure timely access to needed care
  • Performs telephonic, digital, home and/or other site outreach to assess member needs and collaborate with resources
  • Collects, documents, and maintains all member information and care management activities to ensure regulatory compliance
  • Provides education to members and families/caregivers on disease processes, care options, referrals, and healthcare benefits
  • Provides feedback to leadership on opportunities to improve care and quality delivery

Requirements

  • Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
  • 2 – 4 years of related experience
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure required
  • Must reside in Bergen and/or Hudson County to travel for home visits within these regions
  • Travel up to 75% required
  • Ability to perform telephonic, digital, home and/or other site outreach to assess member needs
  • Compliance with current state, federal, and third-party payer regulators
  • Ability to provide education to members and their families/caregivers on disease processes and care plans
