About the role

  • Physical Therapist creating and overseeing treatment plans for individual patients in Polk County. Collaborating with teams and physicians to monitor patient progress and outcomes.

Responsibilities

  • Plans and implements treatment programs for individual patients according to physical therapy principles
  • Develops, implements, monitors, and revises treatment programs for assigned patients
  • Records progress notes and communicates patient progress with team members
  • Confers with physician on patient's progress
  • Maintains equipment and orders supplies; performs related clerical work
  • Provides direct clinical services to families and children within the Early Steps program
  • Participates in education and training to medical students and pediatric residents

Requirements

  • A Bachelor's degree in a health-related field such as anatomy, physiology, biology, chemistry, or physics
  • Graduate from a physical therapy program accredited by the Commission on Accreditation in Physical Therapy Education
  • Florida State Licensure as a Physical Therapist
  • Must be able to drive to locations in Polk and Eastern Hillsborough County
  • Spanish and Bilingual Preferred
  • Knowledge of Florida Early Steps or Early Intervention System of Care Preferred
