About the role
- RN Case Manager coordinating hospice care for patients in their homes in Texas. Focused on comfort, dignity, and quality of life while managing patient care plans and interdisciplinary communication.
Responsibilities
- Start your day visiting patients in their homes, focusing on comfort, dignity, and quality of life
- Assess patient needs, manage symptoms, and adjust plans of care as conditions change
- Coordinate closely with physicians, hospice aides, LPNs, social workers, and chaplains
- Support and educate families, helping them feel confident and cared for
- Document care clearly and efficiently in our EMR, Consolo
- Advocate for your patients while maintaining autonomy in how you structure your day
- Serve as the primary nurse coordinating care for your assigned patients
- Develop, implement, and update individualized plans of care
- Determine visit frequency based on patient acuity and family needs
- Provide guidance to Hospice Aides and LPNs to ensure excellent care
- Communicate changes in condition to physicians and the care team
- Participate in on-call rotation
Requirements
- Active RN license in the state you’ll practice
- Graduate of an accredited nursing program (ADN or BSN)
- 2+ years of RN experience preferred (hospice experience a plus—but not required)
- Strong clinical judgment, compassion, and communication skills
- Reliable transportation, valid driver’s license, auto insurance, and CPR certification
