About the role
- Pediatric Social Work Care Manager collaborating with interdisciplinary teams to evaluate pediatric patients’ needs. Providing psychosocial assessments and developing treatment plans in a medical setting.
Responsibilities
- Provide psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues
- Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team
- Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan
Requirements
- Master's Degree Social Work or Master's Degree Mental Health & Behavioral Medicine required
- 0-1+ years of clinical experience in a medical setting required
- Class D Passenger Vehicle Driver's License required
- Licensed Clinical Social Worker (LCSW), Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC) required
- Pediatric experience strongly preferred
