Responsibilities

  • Coordinate and manage comprehensive care plans for individuals requiring long-term support.
  • Assess member's needs and develop personalized discharge plans.
  • Ensure seamless transitions between care settings to promote optimal health outcomes.
  • Collaborate closely with multidisciplinary teams, including healthcare providers, social workers, and community resources.
  • Facilitate access to Medicaid and other support services.
  • Advocate for members and monitor their progress to ensure quality care delivery.

Requirements

  • Bachelor’s degree in Social Work, Human Services, Nursing, or a related field.
  • Experience in case management within healthcare or social services settings.
  • Knowledge of Medicaid policies and procedures.
  • Relevant experience may substitute for the educational requirement on a year-for-year basis.
  • Master’s degree in Social Work (MSW) or related discipline (preferred).
  • Certification in Case Management (CCM) or equivalent (preferred).
  • Experience working with diverse populations in community-based settings (preferred).
  • Familiarity with electronic health records (EHR) and care coordination software (preferred).
  • Training in trauma-informed care or behavioral health interventions (preferred).
