About the role

  • Family Partner supporting youth and families in Project Permanence program. Collaborate with clinicians and liaisons to assist in treatment goal attainment.

Responsibilities

  • Team with Project Permanence Clinicians and Community Liaisons to support youth and families in meeting treatment goals.
  • Provide insight, expertise and relevant resources to treatment team and families.
  • Assist clients and caregivers in accessing resources in the community.
  • Participate in Project Permanence team meetings and meetings with families in the home or community.
  • Support caregivers and youth by identifying, creating and working with resources to sustain tasks and goals.
  • Document services timely according to Lincoln and county standards for billing Medi-Cal services.

Requirements

  • Bachelor's degree, preferably in psychology or social sciences, or equivalent experience.
  • English/Spanish Bilingual skills highly preferred, but not required.
  • Experience working with youth and/or families, preferably parents.
  • Experience in mental health or social service systems.
  • Valid California driver's license, satisfactory driving record, availability of personal vehicle and personal vehicle insurance coverage.
  • Must meet eligibility requirements to bill for Medi-Cal services.
