About the role

Responsibilities
- Conducting service coordination functions for a defined caseload of individuals
- Facilitates the Person Centered Planning process that documents the member’s preferences and needs
- Conduct assessments, develop comprehensive Person Centered Support Plans (PCSP)
- Interface with Medical Directors and participate in interdisciplinary care rounds
- Engage the member’s circle of support and manage the individual’s physical/behavioral health needs
- Document short and long-term service and support goals
Requirements
- BA/BS degree
- A minimum of 2 years of experience working with a social work agency
- Experience working with older adults in care management preferred
- Experience managing a community and/or facility-based care management case load highly preferred
- BA/BS degree field of study in health care related field preferred
