About the role

  • Behavioral Health Care Manager II managing case management for behavioral health needs. Collaborating with providers and utilizing clinical judgment to support client care plans and access.

Responsibilities

  • Perform case management telephonically and/or by home visits within the scope of licensure
  • Respond to more complex cases and account specific requests
  • Conduct assessments to identify individual needs and develop specific care plans
  • Monitor and evaluate effectiveness of care plans and modify as needed
  • Support member access to appropriate, quality, cost-effective care
  • Coordinate with internal and external resources to meet identified member needs
  • Serve as a resource to other BH Case Managers
  • Participate in cross-functional team projects and initiatives

Requirements

  • MA/MS in social work counseling or a related behavioral health field, or a degree in nursing
  • Minimum of 3 years of clinical experience in social work counseling
  • Current active unrestricted license to practice as a health professional issued by the state of New York
  • Previous experience in case management and telephonic and/or in-person coaching with complex psychiatric/substance abuse and/or medical disorders
  • Managed care experience required
