About the role

  • Clinical Consultant Pharmacist enhancing health through personalized pharmacy services for long-term care. Collaborating with onsite physicians and nursing staff for optimal medication management.

Responsibilities

  • Act as a liaison between long term care homes and retirement communities and CareRx Pharmacy to ensure optimal Resident care, Pharmacy Services, and Clinical support.
  • Travel to LTC homes and retirement communities.
  • Build and maintain relationships within the community to ensure ongoing collaboration and communication between home/community leadership teams and pharmacy teams to develop/maintain strong partnerships.
  • Complete medication reviews in currently serviced retirement communities and long-term care homes serviced by CareRx Pharmacy.
  • Complete all required documentation and billing processes as required by MOHLTC and College of Pharmacists of British Columbia.
  • Provide education to Residents and staff on various health-related topics including the Meds Check program and attend community-based wellness fairs and other events as required.
  • Conduct education sessions within Long Term Care Homes and Retirement Communities within scope of practice.
  • Support CareRx Pharmacy policies and procedures within retirement communities and long-term care homes.
  • Complete medication audits on-site in LTC Homes and Retirement Communities as required.
  • Ensure regular on-site visits are made to all assigned clients.
  • Attend/participate in annual professional conferences and trade shows, when applicable.
  • Participate in the development and implementation of industry-leading clinical pharmacy service, when applicable.
  • Help with pharmacy operations/drug distribution when required.
  • Attend and actively participate in clinical team meetings.

Requirements

  • Insured and currently licensed in good standing with the College of Pharmacists of British Columbia.
  • Proven ability to foster strong partnerships with other members of the healthcare team.
  • Effective organization, planning, and time management skills.
  • Knowledge of Kroll Windows application and Microsoft Office programs.
  • Complete understanding of provincial regulations governing LTC homes and Retirement Communities.
  • Familiar with the provincial standards of practice established for LTC pharmacies.
  • Excellent customer service and interpersonal skills.
  • Excellent time management skills.
  • Skilled in critical thought processes and able to prioritize appropriately.
  • Must demonstrate initiative and creativity.
  • Self-directed requiring minimal supervision and able to work independently.
  • Motivated to achieve results that reflect a high standard of quality.
  • Good listening skills and adaptable to changes in job environment.
  • Dependable and responsible in the execution of job duties.
  • Strong presentation and teaching skills.
  • Must possess a valid driver’s license.
